According to the Institute of Healthcare Improvement, a survey of people in 2012 found that 90% say that talking with their loved ones about end-of-life care is important, but only 27% have actually done so. Sixty % of people say that making sure their family is not burdened by tough decisions is extremely important, but 56% have not communicated their end of life wishes.
This Forum will talk about ways to begin the process of thinking about our own deaths and being able to talk with others about our wishes. Facilitators will be Laura Bange, NP and Ann Schuur, LISW-S.
